The processes by which organizations implement clinical quality improvement have received relatively little attention. This paper extends previous work by proposing a model that focuses on the process by which an organization can improve its quality of care. The author used an inductive case study method to develop a conceptual model of leadership practices that support the development of principles and practices of quality improvement within clinical settings. The proposed model focuses on leadership practices in three key areas: 1) changing cognitive frames 2) building organizational supports and capabilities, 3) diffusing and building competencies within the technical core. Both policy makers and organizational leaders can use the proposed model to develop strategies and incentives to develop effective chronic care delivery systems.
